Enroll

Please complete the following application form.  Mural Arts Program staff will follow up with you after your application has been received.  Space is limited.  Additional information, including parent/guardian signatures, may be required before youth can be enrolled in our programs.

You can also download a pdf application here to print and submit by mail or fax.  It is not necessary to complete both the online and paper applications; one or the other is sufficient.
